It is that time of year!  Please take this opportunity to complete your physical information for the 2020-21 School Year!

To participate in Practices and Contests in IHSAA Recognized Sports during the 2020-21 school year on a school-sponsored team, a student who had a prior pre-participation physical examination completed and such examination was completed more than 90 days prior to the first day of official Practice for the student’s sport, may, in lieu of having a 2020-21 Pre-Participation Physical Examination form completed, provide this Health History Update Questionnaire, completed and signed by the student’s parent or guardian, or by the emancipated student. Provided, should any question on this Questionnaire be answered in the affirmative (‘Yes’), then the student must have a 2020-21 Pre-Participation Physical Examination form completed.

Step 1: Login to your Final Forms Account or Create a New Profile if you are NEW to Final Forms.

Step 2:  Fill out the Information if you are NEW.  Confirm the information is correct if you are returning to your profile.

Step 3:  On the final page, “IHSAA PPE Physical (For Physicians)” you will click whether or not your student-athlete had a 2019-20 IHSAA Physical on File and Cleared to play or not.  If the athlete DID NOT have a 2019-20 Physical on file, you will print off the 2020-21 Physical Form to take to the Doctor.  If the athlete DID have a valid 2019-20 IHSAA Athletic Physical on File, it will take you to the IHSAA Health History Update Questionnaire and Consent & Release Certificate  to complete and turn in at the Athletic Office.  You can scan and e-mail to tstenftenagel@gjcs.k12.in.us.

Step 4:  Make sure the Parent/Guardians Sign and the Student-Athlete Signs.

Step 5:  If you have a Physical on File with us or if you had an IHSAA Physical on File with the Middle School, you will need to turn in the IHSAA Health History Update Questionnaire and Consent & Release Certificate  to the Athletic Office.  If you did not have a valid IHSAA Physical for the 2019-20 School Year, Print the IHSAA Physical Form.  We will need a copy of the Completed IHSAA Physical Form in the Athletic Office when it is completed. You can scan and e-mail to tstenftenagel@gjcs.k12.in.us.
